TBR Required Forms to Collect from Vendors

The "Accessibility Statement" form also requires vendors to provide the following (as applicable to the associated product):

Steps for vendor contact

1.       Contact the vendor with an email to include: "We are requesting "Product Name" documentation for accessibility using the attached forms with the TBR adopted WCAG 2.0 AA<https://www.google.com/?gws_rd=ssl#q=wcag+2.0+checklist>, 508 VPAT<https://www.itic.org/policy/accessibility/> and EPub3 accessibility<http://www.idpf.org/accessibility/guidelines/> guidelines and return these completed forms to us at your earliest convenience. Please note the Conformance and Remediation (CR) form will need a completed timeline for accessibility issues/gaps found during review and indicate an estimated conformance date. (i.e. Q4 2017 - see example in the CR form)."

2.       Upon receipt of the forms: verify the Accessibility Statement form questions have been completed and a completed timeline for accessibility issues/gaps found during review exists in the CR form.

3.       Send the forms to your AIMT Webform representative for administration
